We support the coordination of health and wellness activities in local communities and provide ways that the public and service providers can work together. We have focused our activities to support people who are vulnerable to disease and isolation in the community. We do this through two websites, with plans to link them into an exciting joint platform, My Community Directory and My Community Diary. We actively develop and provide practical frameworks, guidelines and resources for the public, community organisations, councils and government to work together with people who are vulnerable.
